The HubSpot lookup by email happens before any record is written, so an existing contact gets the note instead of a duplicate sitting beside it.
The reason a lead was routed appears in their own words, which is what lets a rep disagree with the routing in about ten seconds.
It searches by the email address on the response first and creates a contact only when nothing comes back, so the usual outcome is a note on the record you already have. Somebody who fills the form twice with two different addresses will still become two records, exactly as they would if a rep typed them in. It writes notes rather than editing properties, deals or lifecycle stages.
