Triage looks like labelling and is really reading: the whole comment history first, so a follow-up on an old conversation is not filed as a fresh problem, then a classification that has to hold against rules you wrote. Claude Code reads its context through before it acts, which is what stops a three-message thread being triaged off its opening line. The note it leaves is six lines at most, so the reading matters more than the writing.

Since 08:15 this morning every customer who clicks Pay gets a 500 error page and no order is created. I have tested in three browsers and on two devices, and it reproduces on our staging store as well. We are a florist and this is our peak delivery window, so we are losing roughly 40 orders an hour. Nothing changed on our side except the platform update notice you sent yesterday.

Inventing a plausible new category is the failure mode that quietly makes a triage system useless. The rules you wrote are the entire vocabulary and it stays inside them.
